Written Consent for Participating in World Masters Games 2021 KANSAI

In participating in the World Masters Games 2021 KANSAI (hereinafter referred to as “the Games”), I understand, consent to, and comply with the Participation Terms and Conditions, the Sports Information Guide, the Cancellation Policy, the Personal Information Policy, the Terms of Use of the Entry System, and the World Anti-Doping Rules, and to follow other operational instructions on the Games. I further agree to the following terms:

1. I am fully aware that participation in the Games involves the risk of suffering accidents, missing personal belongings, injury, illness, and other damage and loss due to various causes, and the risk of causing damage and loss to third parties. I will make every effort to avoid those risks.

I agree that bodily injury may be unavoidable due to contact, etc. if it occurs during a competition conducted in the normal range, and in an environment where appropriate competition management is performed. I will not hold the Organiser responsible for the injury.

2. As far as I know, I have no illness or injury that would interfere with my participation in the Games, and I will have a health checkup in advance, consult a doctor, and receive guidance from the doctor before the Games.

3. In participating in the Games, I understand that the Organiser and companies, organisations, and news media recognised by the Organiser may photograph me in and around the venue of the Games, and I agree that the images, photos, etc., including my portraits will be used for any commercial or non-commercial purposes, such as posting, broadcasting, advertisement, and promotion, in any type of media.
Furthermore, I agree that I am not allowed to charge the Organiser for any use of my portraits under any nominal terms.

4. I guarantee that I am an adult, and if not, I am 12 years old or over and that my participation is with parental or guardian relative’s consent. Furthermore, I guarantee that my participation in the Games is with the consent of my legal representative, such as a parental authority, in advance if I am a minor (under the age of 20 in accordance with the Civil Code and the Act on the Protection of Personal Information of Japan), an adult ward, a person under curatorship, or a person under assistance.

5. I (as well as my family, relatives, legal representative, and any participating partner) agree that the Organising Committee of the World Masters Games 2021 Kansai, the prefectural and major city organising committees, and municipalities’ organising committees, the International Masters Games Association (IMGA), and all volunteers, contractors, sponsors, and other related organisations, in the absence of deliberate or serious negligence, are immune from all liability, loss, and damage.
